Terms and Conditions of AlpineGold's Beauty Therapy Membership Program.

ALL MEMBERSHIP AGREEMENTS MUST BE SIGNED AT TIME OF PURCHASE

This facility sells, and buyer, hereafter referred to as member, purchases the use of this facility for a minimum of 6 months.

I agree that this facility offers the services and hours of operation that I desire to help me meet my treatment goals.

I am physically able to receive the treatments and have no limitation either physically or financially that would hinder me from fulfilling this agreement.

I understand that doctor’s notice of physical inability to continue use is required to put my membership on hold for more than two months without penalty. I further understand that a new doctor’s slip is required every 2 months to continue hold status without penalty or membership agreement will resume automatically without further notice to member. A doctor’s notice of physical inability must be submitted to AlpineGold within 7 days of physical inability.

I the member, understand that I cannot transfer this membership to any other person nor is any portion of my membership refundable after three business days of initiation. If treatments have been redeemed within the first three days, a balance due to full price of service(s) will be due immediately upon termination.

I understand that I cannot cancel my membership if the premises should become damaged or destroyed by fire, flood, tornado, or condemned unless there have been no scheduled treatments for more than 60 days. My membership will be considered on a hold status during such time.

I do hereby release this facility and its employees from any claim or cause of action, which may have occurred as a result of any medical problem known or unknown which I presently have or later develop. I understand that the staff employed at this facility are not qualified to make medical assessments of my health or my physical ability to receive treatments and it is my responsibility to check with my physician before starting any treatment program.

I verify that no promises or guarantees, other than those written on this agreement, were made to me by this facility or its employees. I agree to follow the instructional guidelines when given. Failure to do so may result in cancellation of my membership.

I understand that if I fail to pay the amount owed by this agreement, whether I have used the treatments or not, within the time stated, AlpineGold has the right to collect the balance of this agreement and at its option, turn the balance over to collections. I understand I will be responsible to pay all collection fees up to 50% and attorney fees as awarded by the court.

I agree and understand that if I wish to cancel this contract, I must cancel by emailing merritt@alpinegolddayspa.com within three (3) business days of signing this original contract. If I cancel within said three (3) business days, AlpineGold will return, within ten (10) business days from the date on which I gave notice of cancellation, any payments I have made less treatment fees used.

I give permission to AlpineGold to capture my facial image by means of any form of media, including photo or video, and agree that any such media may be used for any and all purposes that AlpineGold sees fit, and without compensation.

EFT CONTRACTS:

This facility sells, and buyer, hereafter referred to as member, purchases the use of this facility for a minimum of 10 EFT payment months. 10 Monthly payments, in addition to first 2 payments submitted at time of set up. Member shall be charged 1st payment in form of service payment and 2nd months payment in the form of an INITIATION FEE at the time of purchase. This Authorization will remain in effect until AlpineGold has received written notification from me of its termination by the 30th day of the month prior to the date I wish to cancel. Cancellations must be emailed to merritt@alpinegolddayspa.com. Until AlpineGold actually receives such a notice, I agree to continue to be charged for up to 10 additional payments. I agree if I cancel my membership before completing 6 months I will pay a $85 early termination fee, and any promotions for 12 month commitment that were received and used must be paid back in full. If the promotion portion was not used, it will be forfeited upon cancellation. I agree that treatment of each such draft, and your rights in respect to it, shall be the same as if it were signed by me.

I agree to pay the monthly fee on the date in which initially signed off each month, as will be automatically transferred from my billing information on file as payment to AlpineGold. Failure to pay monthly dues will result in a $25 returned payment fee charge, to be enacted after 7 days of failed electronic funds transfer payment.
